    Example answer: I was sick maybe two months ago. I had a bad cold.


  1. How often do you get sick?

    Example answer: Not often. Maybe one or two times per year.


Health Overview

  1. Overall, are you in good health? Do you have any health conditions (that you are comfortable sharing)?

    Example answer: Overall, I'm in good health, but I have high blood pressure.


  2. When was the last time you took a blood test?

    Example answer: I took a blood test two years ago. I should book another one soon.


  1. Do you have any allergies?

    Example: Yes. I have seasonal allergies. I'm also allergic to cats.
